Discounts cannot be combined with treatment-credit redemptions or other promotions unless expressly stated. Complimentary enhancements are limited to one eligible selection per billing month. Medical treatments require clinical assessment. Full membership terms will be provided before enrollment. Membership cancellation requires a minimum of 30 days written notice prior to the next billing date. Appointment cancellations require at least 24 hours advance notice; cancellations made with less than 24 hours notice are subject to a $75 cancellation fee.
